Physician’s statement (POC) Passengers using a Portable Oxygen Concentrator (POC) are required to submit the necessary documents and make a request through our reservation center at least 48 hours prior to departure.

U.S. Department of Transportation Service Animal Relief Attestation Form Please fill out the form and submit it to Reservations at least 48 hours before the departure.

Passengers that need to submit a pledge on their health status

Passengers who are 32 weeks to 37 weeks pregnant
Passengers traveling with a portable injector to treat symptoms of diabetes or specific allergies
Passengers who have medical devices implanted (e.g. artificial pacemaker, insulin pump or spinal stimulator)
Passengers suffering form acute infectious diseases or regular infectious diseases
Passengers with cardiovascular diseases, such as myocardial infarction, angina or cardiac insufficiency, or with a history of heart surgery
Passengers with respiratory diseases such as chronic obstructive pulmonary disease, asthma, pneumonia, pulmonary edema and pneumothorax
Passengers with a history of neurological diseases, such as strokes or brain tumors, or who have underwent brain surgery
Passengers who underwent surgery in the last month
Passengers who are currently admitted to a hospital or are suffering from diseases requiring ongoing treatment
Passengers with unmoderated mental illness
